Position: Chronic Care Management Coordinator (LPN)

Key Responsibilities:

Manage a patient census with a focus on resolving gaps in both clinical and non-clinical care.
Identify and coordinate community resources that can enhance patient care and support overall health management.
Provide patient education to improve health literacy and empower patients in managing their chronic conditions.
Oversee medication management by identifying concerns, ensuring medication reconciliation, promoting adherence, and coordinating refills.
Ensure timely delivery of services to patients, including home health, durable medical equipment (DME), home infusion, and other critical needs.
Address patient questions, fostering open communication to better understand their needs.
Support referrals and assist with appointment scheduling.
Additional Requirements:

Active New York LPN License
Comfortable with Microsoft Office Suite
Excellent written and verbal communication skills
Preferred Qualifications:

Epic experience
Bilingual in English and Spanish
Active California LPN License
Remote Work Requirements:

Must have reliable high-speed Wi-Fi
Must have a dedicated home office or a HIPAA-compliant workspace
Physical Requirements:

This position is sedentary and will require extended periods of sitting.
Ability to speak clearly and listen attentively during telephone conversations for extended periods.
Ability to take notes in real-time while engaging in phone conversations.
